I had a rescue cat in foster because he didn't use his litter box. He peed outside the litter box, nearly always. After coming to us in just a matter of days I was able to determine the poor guy had an urinary tract infection. He urinated outside of his box, because it was painful to go pee and he associated the pain with the litter box. Of course he still had to urinate and would try to find somewhere to pee where it wouldn't hurt which was about everywhere imaginable in the house.
Once he was diagnosed and treated with antibiotics, isolated with his litter box in a small room(for a brief period of time), he resumed his usage of the litter box with no problems at all! He went on to a wonderful home and to this day many many years later, he is faithful to his litter box.
Cats can have medical problems which cause them to pee outside the litter box. You should always have your cat vet checked when you notice urination outside the box.
